Data Systems Used by Early Intervention Evaluators

View the established condition database with information about medical diagnosis that qualify a child to receive Early Intervention Colorado services.

New in 2022, the Early Intervention Referral Case is used to house all information for a child referred to Early Intervention Colorado from the point of initial referral through eligibility determination. 

The EI Referral Case includes child and caregiver demographic information, prior services, attempts to contact, health information, DHS involvement, if any, intake questionnaire, eligibility information, referral source, evaluation scheduling, status, contact notes and documents gathered during intake. Only eligible children will move forward to receive an EI Case.
